How to present them - however the customer wants them.
You should have a range of options priced accordingly.
Price/markup - whatever means that you are making a "decent" profit. Only you can decide what that will be.
Google portrait photographers and look at others price lists to get some sort of basic idea.

The way I present my pictures (usually same day of shooting or next day) is on a slideshow. The purpose of the slideshow is to give them an idea of what the pictures look like. Some times they tell me which the prefer and then I just work on those.
If I don't get feedback, then I go through all the pictures and begin editing (cropping, sephia, B/W, partial color, effects, etc) and once done I put them all in on gallery where they order their prints. Each picture can be ordered as 4x6, 5x7, 8x10, 8x11, etc and each size has a different price. Of course, as you can see I don't have packages but in some cases I just charge a driving fee or something like that and then they order as much or as little as their budget allows them
Good luck
